In order to provide you and your child with the maximum assistance, it is important that this practice have as much information as possible concerning the developmental history of your child. This includes information related to the difficulties experienced by your child as well as the results of any medical, psychological or any other professional testing. In addition at times it is helpful to record parts of therapy sessions or take photos within therapy/assessments for record keeping and training of parents/teachers/therapists. These videos or photos will not be shared with anyone outside of this practice without your express written consent. As far as possible any identifying information is avoided in these media files. To obtain reports on such tests and to forward any information (to your doctor, the school, other therapists, other professionals), we require your permission in writing. We assure you that this is purely for your child’s best interests and that all information on your family and child will be treated as strictly confidential.

Unencrypted email is not a secure form of communication. While our email is secure all there is some risk that any individually identifiable health information and other sensitive or confidential information that may be contained in such email may be misdirected, disclosed to or intercepted by unauthorised third parties. However, you may consent to receive email from us regarding your treatment. We will use the minimum necessary amount of protected health information in any communication. In this practice the SpecialT web application will be used as a means of communication and storage for specific OT activities for your child. I, have been informed of the risks of during the COVID-19 pandemic. I have been informed of the safety precautions in place. I am aware of the potential risks to me/my child during the occupational therapy assessment/treatment/care, and I have made an informed decision to continue with my treatment.

I hereby give consent for my child to participate in Occupational Therapy. I understand that although the necessary precautions for my child’s safety and well being will be adhered to at all times, I fully accept that all activities will be undertaken at my child’s own risk. I confirm that I have read, understand and agree to the conditions outlined in the conditions of therapy document, as listed above, and have asked all relevant questions which have been answered to my satisfaction. I hereby undertake on behalf of myself, my executors, my spouse and my child to give full indemnity to the therapists working with my child and the establishment in which s/he receives Occupational Therapy. Full indemnity will include any claim against the therapist or establishment for a loss of any nature or any injury of any nature or severity. I understand and accept the conditions of therapy and indemnity for my child as stated above.
